THE FOLLOWING DOCUMENTS MUST BE SUBMITTED ALONG WITH THE FORMS:
A COPY OF THE DATE OF BIRTH CERTIFICATE FROM THE MUNICIPAL CORPORATION / CANTONMENT BOARD / BAPTISMAL RECORDS.
ALL ORIGINAL CERTIFICATES MUST BE SHOWN AT THE TIME OF SUBMISSION OF THE FORM. HOWEVER, IF THE INFORMATION SUBMITTED IS FOUND INCORRECT ON VERIFICATION THE FORM WILL BE REJECTED.
ALL CHRISTIAN APPLICANTS MUST SUBMIT A PHOTOCOPY OF THE BAPTISM CERTIFICATE ALONG WITH THE BIRTH CERTIFICATE OF THE MUNICIPAL CORPORATION / CANTONMENT BOARD AND ALSO A LETTER FROM THE PARISH PRIEST.
FOR ALL S. C. / S. T. OR BACKWARD CLASS APPLICANTS; A CERTIFICATE FROM THE DISTRICT MAGISTRATE OR DESIGNATED AUTHORITY OF THE DISTRICT MAGISTRATE MUST ALSO BE ATTACHED.
THE APPLICATION FORM TO BE PRINTED ON A SINGLE SHEET OF A4 SIZE AND DULY SIGNED BY BOTH PARENTS.

Payment made for the registration form shall not be refunded.
Both the pages of the registration form to be printed on a single sheet and to be signed by both parents.
If the registration form is printed on two sheets then the same may be rejected.
If the registration form is not having Computer Generated Form No. the form shall be treated as invalid.
Child name and mobile no. entered once at the time of user registration shall be locked and entered automatically on the registration form. no change is allowed at the time of filling the form therefore parents are advised to carefully enter the child name and Mobile No. of Father/Gaurdian/Mother.
The registration form has to be filled online only, no entry on the registration form is to be made by pen after printing of the form.
Cutting or overwriting on the printed form is not allowed.
INCOMPLETE OR INCORRECT FORMS WILL BE REJECTED.
The Registerd mobile no may be used to fill a single registration form.
DUE TO THE LIMITED NUMBER OF SEATS AVAILABLE AND HEAVY RUSH, ALL APPLICANTS REGISTERING FOR NURSERY CANNOT BE ACCOMMODATED. PLEASE APPLY IN OTHER SCHOOLS ALSO.
